Fans of Grey's Anatomy have somewhat gotten used to the blood and gore on the show, and have been exposed to some crazy situations.
From an early episode featuring two strangers impaled together by the same pole, to victims of a lion on the loose and a man with a bomb inside his body, fans have seen it all, but somehow, the show still manages to catch them by surprise.
The plane crash
Grey's Anatomy's biggest WTF moment has to go to the episode that seemed to change everything about the show: the plane crash. In the eighth season finale, six Grey's Anatomy doctors are involved in a traumatic aviation accident that leaves them fighting for their lives in the wilderness. Fans who had somewhat gotten used to the horror depicted in the operating room were in no way prepared to see fan-favorite Lexie crushed to death under plane debris and later eaten by wolves.
George's death
Fans are usually less affected by random character deaths, as they happen often and usually before viewers have the chance to grow attached to them. Understandably, viewers don't feel too much panic when a severely disfigured bus accident victim is brought in, however, he communicates with Meredith to reveal that he was none other than beloved George O'Malley. Moments after this revelation, George flatlines before fans had fully processed who he was.
Derek's death
The next WTF moment was not only unexpected, but fans felt it was so uncalled for.
