It is actually a scene from the season 2 episode, Breakdown, where Dr. Murphy, played by Freddie Highmore, is removed from a major operation by Dr. Han, portrayed by Daniel Dae Kim, and he does not take the blow lightly.
Weird, considering he is a surgeon (if you haven't guessed it already), and they are supposed to be rather cool-headed.
Why has the scene become a meme now, four years later after its original release? Well, memes have a science of their own, and we are not blessed with the sacred knowledge of how they appear.
