The appeal of a historical setting comes first and foremost from its aesthetics: it's epic, grand, a bit dark, and seemingly slightly unsanitary, definitely not very practical, but still alluring. Different showrunners take different approaches to realism.
Some prefer to go for the edgy and gritty vibe, dirty and unsettling, while others tone it down a bit and settle on the bare minimum of believability, using the historical events merely as a setting for some drama that can very well happen even in our modern days.
