The writers of Grey's Anatomy have rarely failed to shock fans with their plot twists, and it's no surprise that the medical drama has remained a fan favorite and household name for years on television.
However, there have been some plot twists that top all the rest, and the first twist is set up from the very beginning.
Meredith and Derek in episode 1
During episode 1, fans are introduced to the main character, Meredith Grey who meets Derek at a bar. Their interaction leads to a one-night stand and although Meredith has no intention of seeing him again, the writers of the show have other plans.
She arrives at Seattle Grace Hospital where she works only to run into Derek, who had just started as an attending at the same hospital. Cue another plot twist: Addison. Fans, and Meredith, are shocked to discover in the second episode that Derek is actually married.
Denny Duquette
The charming, handsome Denny Duquette arrived at the hospital needing a heart transplant and it soon felt like he was becoming a main character. He and Izzie Stevens struck up a sweet, genuine romance.
Fans were rooting for the pair, and things seemed to be working out for them when he successfully received a heart transplant and then proposed to Izzie, but in a sad twist of fate, Denny dies from a blood clot as Izzie arrives to show him her prom dress.
Not only did this put Denny Duquette on the list of most devastating character deaths, but it also was one of the biggest shocks to fans who believed Grey's would let him live.
Another character death
The death of another beloved character sits at number 3, but this twist was even more unexpected. Fans don't feel any more anguish than usual when a bus accident victim is brought into the hospital. His disfigurement leaves him unidentifiable, and Meredith tries to comfort the suffering John Doe, who communicates with her by writing on her hand with his finger. He writes "007," and at first Meredith is confused. Suddenly it all clicks, it's George O'Malley. The doctors race to save their friend, but he flatlines shortly after they learn his identity.
Gary Clark storyline
After the death of his wife, fans think they have seen the last of Gary Clark, but they were very wrong. The devastated husband storms the hospital on a shooting spree seeking revenge. 18 people were shot and 11 died from their injuries, including the main characters. This turn of events left viewers stunned, nobody could have anticipated the events that took place.
The death of McDreamy
Finally, another death that practically tore fans' hearts out, was the death of Derek Shepherd. The much loved McDreamy was in his car after helping a family who had crashed, when he was unexpectedly taken out by another car. After his shocking accident, fans hold onto hope that the nearby hospital can save him, but they neglect to give him an MRI and he is left brain dead.
